If you meet these qualifications, we would love to meet you:* Registered Nurse with current licensure to practice nursing in the state.

• Registered Nurse with minimum (1) year of experience.
• Must have a valid driver's license and reliable transportation

Key Responsibilities:

As a RN Field Nurse, you will:* Assessment of the status of nursing needs of assigned patients, implements the individualized patient care

• plan, evaluates the effectiveness of the care plan, and modifies as appropriate.
• Responsible for promoting and maintaining optimal client care.
• responses to actual or potential health problems.
• Makes the initial nursing evaluation visit.
• Re-evaluates the client's nursing needs as required.
• Initiates the plan of care and necessary revisions.
• Counsels and instructs the client and family in meeting nursing and related needs.
