Position Overview

The Occupational Therapist (OT) will evaluate patients and provide skilled occupational therapy services to improve functional independence, daily living skills, and safety in the home environment. This role focuses on helping patients regain or maintain the ability to perform daily activities.

Responsibilities

• Perform occupational therapy evaluations and develop individualized treatment plans
• Provide skilled therapy interventions to improve activities of daily living (ADLs) and functional mobility
• Educate patients and caregivers on safety, adaptive equipment, and home exercise programs
• Collaborate with physicians, nurses, and interdisciplinary team members
• Document visits, progress, and outcomes accurately in the EMR system
• Participate in case conferences and care coordination
• Ensure compliance with clinical standards, Medicare guidelines, and agency policies
• Supervise Certified Occupational Therapy Assistants (COTAs) as needed

Qualifications

• Active Occupational Therapist (OT) license in the state of California
• Minimum 1 year of experience as an Occupational Therapist (Home Health preferred)
• Strong clinical, communication, and documentation skills
• Ability to work independently in the field
• Valid driver’s license and reliable transportation

Preferred qualifications:

• Home health or post\-acute care experience
• Experience with OASIS and EMR systems
• Bilingual candidates are a plus
