RN Case Manager/Supervisor

A Plus Home Care, Inc. is searching for a dependable, dedicated and hard-working Registered Nurse for our RN Nurse Supervisor position that is excited about being challenged and can adapt and flex to a growing, changing company.

The position has flexible hours ranging from 15-30 hours per week depending on your schedule and availability

We offer flexible hours, the ability to create your own schedule, and an option to do some work remotely from a computer.

Duties:

1. Conduct assessments of client needs

2. Oversight and supervision of field staff

3. Meeting with new clients to open case files and complete new open paperwork

4. Meeting with existing clients on a consistent basis to review cares, services and paperwork

5. Creation of client Care Plans to direct services

6. Training of field staff on specific client cares

7. Charting of progress notes based upon scheduled visits

8. Collaboration and care coordination with office staff and case managers

9. Review of field staff to assess competency and knowledge

10. Be a resource for field staff regarding client questions and concerns

The ideal candidate must have:

Independence/time management – much of this position is done at home or in the field so Nurse Supervisor need to be self-starters that can stay on track and work autonomously

Reliability – because this position is more autonomous, once trained in, our company needs to feel confident that the job is being done in a timely fashion and according to policy and procedure

Team player attitude – possess the understanding of a small office environment and willingness to help in areas that may not necessarily be “in the job description” and willingness to jump in to other areas of business on occasion even if unfamiliar with them

Positive attitude – difficult situations come up daily and you must be willing to challenge yourself to handle it effectively and appropriately with professionalism and positivity

Organizational skills – must be able to keep track of client appointments, scheduling, charting, and stay on top of policy & protocol changes

Current RN license in the state of WI required. Current CPR certification required. Home Care experience of at least 1 year strongly preferred.
