Nurse Practitioner

Job Summary

We are seeking a Family Nurse Practitioner to provide thoughtful, evidence-informed care across patient settings (pediatrics). The role includes assessing health needs, developing care plans, and coordinating with healthcare professionals.

Duties

• Conduct patient assessments, physical examinations, and diagnostic evaluations; review vital signs, medical records, and relevant test results.
• Develop care plans, prescribe and administer medications within applicable scope and Drug Enforcement Administration (DEA) regulations, and provide patient education and health coaching.
• Coordinate referrals, case management, discharge planning, and follow-up across primary care, acute care, and post-acute settings.
• Document care accurately in electronic medical record (EMR) and electronic health record (EHR) systems, following Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Collaborate with healthcare professionals on triage, infection control, and safe, patient-centered care.

Experience

• Current Nurse Practitioner credentials and authorization to practice are required; applicable collaborative practice agreements must be maintained.
• Demonstrated patient care experience, with strengths in patient assessment, medical documentation, medication administration, and client communication.
• Familiarity with EMR/EHR platforms.
• Relevant experience may include primary care, internal medicine, pediatrics, geriatrics, emergency medicine, hospital medicine, critical or intensive care, dialysis, gastroenterology, behavioral health, occupational health, hospice and palliative medicine, or outpatient and inpatient care.

Pay: $55.00 - $65.00 per hour

Expected hours: 24.0 – 40.0 per week
